Your sign-up form is how customers join your loyalty programme. They fill it in, and their loyalty card is ready to add to Apple Wallet or Google Wallet straight away.
This guide covers how to create a form, what you can collect, and the different ways to share it.
Create your form
Head to Forms and create a new form. Each form is linked to one loyalty card, so every card you create needs its own form.
1. Choose your card
Under Select Card, pick the loyalty card customers will receive when they sign up.
2. Add your form details
Form Description: A short welcome message shown at the top of the form. You can use emojis and bold text. Press Shift + Enter to start a new line.
Submit Button Text: Make it your own, such as "Join now" or "Get my card".
Background and Text Colour: Match your brand, and make sure the text stands out clearly from the background.
Unique Field: Choose Email or Phone. Loyaltify uses it to recognise customers, so the same person can't join the same card twice.
Your logo comes from Store Settings, so it appears on the form automatically.
3. Choose what to collect
Turn on the details you'd like to ask for:
Name: Always included. Customers enter their full name.
Email and Phone: At least one is required, and it must include the one you chose as your Unique Field.
Birthday: Optional. Add it if you'd like to send birthday rewards.
Marketing Consent: A tick box customers can choose to tick. Only customers who tick it receive your marketing push notifications. If you remove it from the form, you won't be able to send marketing messages to people who sign up through it.
Customers always accept the Terms of Service when they sign up. To link your own terms, add them in your card's Advanced Settings.
💡 Tip: Want to explain what customers get for opting in? Click the pencil next to the marketing consent text to edit the wording, for example "Send me exclusive offers and birthday treats".
4. Ask your own questions (Growth and Advanced plans)
Add custom questions to learn more about your customers. You can choose from these types:
Text: Short open answers, such as "How did you find us?"
Date: A date, such as an anniversary.
Dropdown and Single Choice: One answer from a list, such as "How often do you visit us?"
Multiple Choice: Several answers from a list, such as "What do you enjoy at our café?"
Hidden (From URL): Not shown to customers. It saves a value from your form link, which is handy for tracking which ad or campaign brought someone in.
Your questions are saved in one shared list, so you can reuse them across forms. You'll find each customer's answers under Edit Customer and in your customer export.
💡 Tip: Want to know where your sign-ups come from, such as a branch or a partner? Add a question like "Where did you hear about us?" to each form.
5. Check the success screen
After signing up, customers see a confirmation screen with a button to add their card to Apple Wallet or Google Wallet. This screen appears in the customer's phone language and can't be edited.
To see what your customers get after that, and how to help them, see How customers get and use their loyalty card.
Share your form
Once your form is ready, there are plenty of ways to get it in front of customers:
Copy the link: Click the pencil next to your form on the Forms page to find its link. Share it on WhatsApp, by email, on social media or on your website.
Download the QR code: Use the ⋮ menu next to your form to download it as a PNG, ready for your own menus and posters.
Print a flyer: Loyaltify creates one for you automatically. See Flyers.
Add it to your website: A button or link to your form works best on phones. You can also embed the form on a page of its own.
Want to embed the form, send personal links, track which ad brought someone in or use your own web address (such as join.yourbrand.com, €99 per domain)? See How to share your form online.
Good to know
Your link stays the same. Editing a form, your card design or your Store Settings doesn't change the form link or QR code, so printed flyers keep working. Deleting a form does break them, though.
Anyone with the link can join. There's no approval step. If you want to stop new sign-ups, delete the form.
Already a member? If someone fills in the form again with the same email, they get their existing card by email. Their saved details don't change, so to update them, use Edit Customer.
Telling forms apart: Forms can't be renamed, but each one shows its ID next to it.
Answers don't add points. Filling in the form doesn't give customers stamps or points on its own. Use your card's initial balance for a welcome bonus.
Something not working?
You can't type into the form on the Forms page: That's only a preview. Open your form link to test it properly.
The form looks blank or the button is hard to find: Check your text colour isn't the same as your background, such as white on white.
The card downloads as a file instead of opening Wallet: The form was opened on a computer or inside another app. Open the link in your phone's own browser, such as Safari or Chrome.
An error appears when you save the form: Make sure every Single Choice question has at least two options, then refresh the page (Ctrl + Shift + R, or Cmd + Shift + R on a Mac) and try again.
Dates on the card show as MM/DD/YYYY: Add your business address under Store Settings → Branches, then create a new form.
